Patient Care Coordinator or Nurse Quality Assurance

• Facilitates the care team in establishing objectives for client satisfaction
and best practices, scans documents into patient charts, manages patient
accounts discreetly, and ensures compliance with regulatory and
professional standards.
• Assists with the processing of in-home patient care requests.
• Reviews and transfers medical charts and notes into the system
• Prefilling OASIS (using kinnser) also includes creating an order specific to
the type of OASIS (e.g. Start of Care Order), initial authorization request and
CMS 485 (for SOC,ROC, REC only).
• Checking Admission Documents: Admission Agreement, POLST, Advance
Directives (Living Will, Power of Attorney, DNR). NOMNC Forms for
Discharge, and should be uploaded to the patient's thread on Carestitich.
• Uses diagnostic codes in translating written descriptions of diseases/
illnesses/injuries.
• Encodes physician's assessments and recommendations.
• Ensuring a detailed wound description is included in the Wound Care Chart,
including pictures of each wound, etiology, size and treatment.
• Generates and submits daily/weekly reports of clients
