I am a Doctor of Medicine graduate who completed my post-graduate internship at Southwestern University Medical Center, Cebu City, Philippine. I rotated across Internal Medicine, OB-GYN, Pediatrics, Surgery, Ophthalmology, Family Medicine, Radiology, and Pathology, where I personally managed patients from history taking and physical examination to assessment and treatment planning under attending supervision.
Clinical documentation was part of my daily routine. I wrote SOAP notes, H&P reports, progress notes, discharge summaries, and operative notes directly into the hospital's EMR, entered records in real time during rounds, and reviewed every chart for completeness before endorsing to the attending physician. I assisted in major surgical cases including laparoscopic cholecystectomy and open appendectomy, and personally handled deliveries with complete labor and postpartum documentation.
Having personally examined patients, formulated assessments, and written treatment plans, I bring a level of clinical depth to documentation that reflects genuine medical understanding. Outside of clinical work, I co-authored a peer-reviewed scientific paper cited in international literature and hold a HIPAA certification.
